หมายเหตุ:

• โปรแกรมนี้ไม่สามารถใช้ได้กับเด็กอายุต่ำกว่า 15 ปี  และหญิงตั้งครรภ์
• ราคาดังกล่าวรวมค่าแพทย์และค่าบริการโรงพยาบาล
• ในกรณีที่ลูกค้ามีการรับบริการเพิ่มเติมนอกเหนือจากในโปรแกรมดังกล่าว ลูกค้าจะมีค่าใช้จ่ายเพิ่มเติม
• เนื่องจากสินค้าและบริการเป็น โปรแกรมตรวจสุขภาพ จึงไม่สามารถยกเลิกหรือขอคืนเงินเป็นบางส่วนของสินค้าได้

• ไม่สามารถออกใบเสร็จของทางโรงพยาบาลสมิติเวชได้ จนกว่าจะมาเข้ารับบริการ
• ราคาดังกล่าวขอสงวนสิทธิ์สำหรับชาวไทยและชาวต่างชาติที่อาศัยอยู่ในประเทศไทยเท่านั้น

• This vaccine program cannot be administered to children under 15 years old or pregnant women
• Price includes doctor’s fees and outpatient service fees.
• Services received in addition to those specified in the program are subject to additional costs.
• Products and services constitute one health examination program; cancellation or refund of any individual part of the program is not permissible.
• Coupon is not refundable after redemption.
• Samitivej Hospital will issue a receipt after the customer has received the services at the hospital.
