• ในกรณีที่ลูกค้ามีการรับบริการเพิ่มเติมนอกเหนือจากในโปรแกรมดังกล่าว ลูกค้าจะมีค่าใช้จ่ายเพิ่มเติม

• เนื่องจากสินค้าและบริการเป็น โปรแกรมตรวจสุขภาพ จึงไม่สามารถยกเลิกหรือขอคืนเงินเป็นบางส่วนของสินค้าได้

• ไม่สามารถคืนเงินได้ในกรณีที่มาใช้บริการแล้ว

• ไม่สามารถออกใบเสร็จของทางโรงพยาบาลสมิติเวชได้จนกว่าจะมาเข้ารับบริการ
